CMS Just Launched a $50B Program for Rural Health:
Here’s What States Need to Know
Applications are due this November. Here's how states can prepare—and how VSM can help.
CMS just unveiled a new opportunity that could reshape the future of healthcare in rural America.
The Rural Health Transformation (RHT) Program, authorized by the One Big Beautiful Bill Act, will distribute $50 billion over five years to states committed to improving rural health systems. From care access and workforce development to tech-enabled innovation and value-based care models, the RHT Program is designed to support strategic, system-wide improvements.
But this isn't a wait-and-see opportunity. States must act quickly.
Key Dates:
📅 Expected mid-September 2025: Notice of Funding Opportunity (NOFO)
📅 Early November 2025: Application Deadline
📅 December 31, 2025: Awards Announced
How Can States Use Rural Health Transformation (RHT) Funds?
Eligible states (limited to the 50 U.S. states—territories are excluded) must select at least three use categories for their proposal.
Options include:
Chronic disease management and preventive health programs
Recruitment and retention of rural health workforce
Adoption of advanced technologies like AI, telehealth, and remote monitoring
Enhancing cybersecurity and health IT infrastructure
Building or restructuring rural care delivery systems
Expanding mental health and SUD services
Developing value-based care or ACO models
This isn’t about patching holes—it’s about long-term transformation.
How VSM Can Help Your State Win—and Deliver
At Value Services Management (VSM), we specialize in exactly the kinds of solutions this funding supports. Our team manages ACOs and value-based care initiatives in rural and underserved markets across the country—so we understand the real-world challenges of access, sustainability, workforce strain, and care fragmentation.
Here’s what we bring to the table:
ACO Infrastructure & Strategy – We’ve built ACOs from the ground up and supported long-term success in the Medicare Shared Savings Program (MSSP), REACH, and other models.
Workforce Pipeline Support – We help provider groups restructure delivery to practice at top of license and attract new talent to rural markets.
Tech-Enabled Care Models – From telehealth and AI to predictive analytics and remote monitoring, we help implement solutions that actually work.
Application Guidance – We can help your state identify priorities, structure a compliant proposal, and write a competitive application that demonstrates both impact and sustainability.
Whether you’re already forming a response team or just starting to explore options, now is the time to plan.
