Supporting Provider Engagement & Performance in Value-Based Care
Position Overview
The Provider Partnership Representative plays a critical role in supporting Accountable Care Organization (ACO) performance under the CMS Medicare Shared Savings Program (MSSP). This role serves as the primary liaison between the ACO and participating providers, focusing on relationship-building, engagement, and performance improvement.
By aligning clinical, operational, and data-driven insights, the Provider Partnership Representative helps ensure providers understand their performance metrics, adopt best practices, and remain actively engaged in initiatives that support shared savings and quality outcomes.
Core Responsibilities
The Provider Partnership Representative is responsible for fostering strong, trusted relationships across the provider network while supporting measurable performance improvement.
Key responsibilities include:
Building and maintaining ongoing relationships with participating providers, practice leaders, and care teams
Reviewing provider-level quality, cost, and utilization data to identify trends, gaps, and improvement opportunities
Facilitating discussions around care coordination strategies, workflow optimization, and best practices tied to MSSP performance
Delivering consistent updates and resources related to ACO initiatives, CMS program requirements, and measure changes
Partnering with internal teams to align provider outreach, interventions, and support strategies
Summarizing provider engagement outcomes and performance findings for ACO leadership and governance committees
Systems, Data & Access
This role is granted access to a wide range of systems and data resources required to support organizational operations and performance analysis.
Authorized access may include, but is not limited to:
Salesforce
SharePoint
Inky Secure Email Encryption
Extracted CMS claims data
Health Information Exchanges (HIE)
Lab extracts
Participating provider EMRs
These tools enable effective workflow management, data-driven decision-making, and meaningful contributions to both operational and clinical effectiveness.
Key Performance Indicators
Success in the Provider Partnership Representative role is measured through outcomes that directly impact ACO performance, including:
Provider engagement and satisfaction levels
Improvement in quality and cost metrics
Timely completion of provider performance reviews
Provider participation rates in ACO initiatives, education, and learning sessions
Qualifications & Experience
The ideal candidate brings experience in value-based care environments and a strong understanding of CMS programs.
Preferred qualifications include:
Bachelor’s degree in healthcare administration, nursing, public health, or a related field(Master’s degree preferred)
3–5+ years of experience in ACOs, value-based care models, or provider network management
Working knowledge of CMS MSSP program structure, quality measures, and attribution methodology
Strong communication, data interpretation, and relationship management skills
